Cheshire and Merseyside Health and Care Partnership
The partnership:
- is a collection of NHS, local authorities, volunteer groups, the community sector, faith groups and social enterprise organisations from across the 9 local authority areas that make up Cheshire and Merseyside
- features place-based care tailored to the needs of the community and local neighbourhoods
- addresses the significant health inequalities across Cheshire and Merseyside and removes barriers that stop people accessing services, such as distance, education, opportunity, and other cultural and social barriers
- acknowledges that they cannot focus on health and care alone – they work with other public services (such as education, police, housing and community organisations) to truly transform health and wellbeing across the region.
